A. Membership Enrollment and Payment
Agreement: This agreement is between the named patient (Member) and Dr. Z.R. Khan's Practice (The Practice).
Payment Obligation: The Member agrees to pay the prescribed quarterly fee via Bank Transfer on the 1st of the month specified in the plan.
Payment Failure: Membership benefits, including access to appointments and discounts, will be suspended immediately upon failure to receive the quarterly payment. Benefits will only be reinstated once all outstanding fees are settled.
Annual Commitment: The Membership is based on a minimum 12-month commitment. While fees are paid quarterly, they cover services and discounts spread over a full year.
B. Included Services and Exclusions
Non-Transferable Services: All included appointments (check-ups, hygiene, X-rays) must be used within the 12-month period and are non-transferable to any other person.
Use It or Lose It: Unused included services (e.g., a missed check-up or hygiene session) within the 12-month period will not be carried over to the next year and are not refundable.
Treatment Discount Eligibility: The advertised discount (10% or 15%) applies only to treatments performed by dentists at Dr. Z.R. Khan's Dental Practice. All discounts are applicable only after continuous, paid membership (6 months for the Essential and Family Plans and 3 months for the Premium Plan).
This waiting period will be applicable to new members and past members who were unsuccessful in renewing their membership within 30 days of completion of the previous 12-month membership period.
Exclusions: The membership plan explicitly excludes the following:
Laboratory fees (e.g. crowns, dentures, implants).
Specialist treatments (e.g. advanced periodontal procedures, complex oral surgery, orthodontics).
Sedation fees.
Medications or prescriptions.
Referrals to other specialists.
Emergency Assessment: The included Free Emergency Assessment covers the initial examination and any necessary localized X-rays only. Any subsequent treatment, such as a temporary filling or root canal access, will be charged separately, less the membership discount.
C. Cancellation and Termination
Cancellation by Member (Within 14 Days): A Member may cancel the agreement within 14 days of enrollment. If any included services have been redeemed, the Member must pay the full retail price for those services.
Cancellation by Member (After 14 Days): If the Member cancels after 14 days and before the end of the 12-month commitment, they are immediately liable to pay the full retail difference between the membership fees paid to date and the full retail price of any services already redeemed.
Example: If you paid 10,000 PKR in membership fees but used services worth 20,000 PKR retail, you must pay the 10,000 PKR difference upon cancellation.
Cancellation by Practice: The Practice reserves the right to terminate the membership without refund if the Member fails to adhere to practice policies, exhibits aggressive behavior, or repeatedly fails to attend scheduled appointments without adequate notice.
D. General Policies
Fee Review: The Practice reserves the right to review and adjust the monthly membership fee annually. Members will be given a minimum of 30 days' written notice of any fee change.
Missed Appointments: Appointments included in the plan (check-ups/hygiene) that are missed without 24 hours' notice will be counted as used and will not be rescheduled or refunded.
